Why Can’t I Lose Weight? A Functional Medicine Guide to Metabolism, Labs, and Weight Loss Resistance

Research from NIH metabolism researcher Kevin Hall and colleagues shows that weight loss is often followed by biological adaptations—including changes in energy expenditure and appetite—that can make continued weight loss and long-term maintenance increasingly difficult (PMID: 29156185).

This doesn’t mean calories don’t matter. They absolutely do.

Ultimately, losing body fat requires an energy deficit. But the human body is not a static calorie calculator. As body weight changes, energy expenditure changes. Hunger can change. Activity can change. Sleep, medications, medical conditions, muscle mass, food environment, stress, and lifestyle can all influence how easy—or difficult—it is to consistently maintain that deficit.

That distinction matters.

First, What Is Metabolism?

A simple way to think about metabolism is:

Fuel → Metabolism → Output

Fuel is the energy you consume through food and drink.

Metabolism is the collection of processes your body uses to convert and use that energy.

Output is the energy your body expends each day.

That daily energy expenditure can be broken down into four practical categories:

1. Resting Metabolic Rate (RMR)
The energy required simply to keep you alive—powering your brain, heart, organs, breathing, circulation, cellular processes, and other basic functions. For most people, this represents the largest portion of daily energy expenditure.

2. Thermic Effect of Food (TEF)
It actually costs energy to digest, absorb, and process the food you eat. This is known as the thermic effect of food.

3. Non-Exercise Activity Thermogenesis (NEAT)
This is all the movement you do that isn’t intentional exercise—walking around your house or workplace, taking the stairs, standing, doing chores, fidgeting, and simply moving throughout the day.

4. Exercise Activity
This is intentional physical activity such as resistance training, running, cycling, sports, or structured workouts.

Together, these make up the majority of the energy your body expends each day. Importantly, these components are not identical from person to person and can change with body size, body composition, food intake, activity level, and weight loss itself (PMID: 15534426).

So when someone says, “I have a slow metabolism,” the reality is usually more complicated.

Your metabolism isn’t one single switch that is either fast or slow. Your total daily energy expenditure is the result of multiple interacting components—and some are more modifiable than others.

That doesn’t mean your metabolism is permanently “damaged.” But understanding these components can help us identify why weight loss may be more difficult than expected and, more importantly, where we can intervene.

Weight Loss Resistance Is Usually Not One Thing

Most people want one answer.

“Is it my hormones?”

“Is it my thyroid?”

“Is it my gut?”

“Is it insulin resistance?”

“Is it stress?”

“Is it genetics?”

The honest answer is that there is rarely one universal explanation.

Weight is influenced by a complex interaction between energy intake, physical activity, appetite, sleep, medications, medical conditions, genetics, environment, and behavior. And during weight loss itself, the body can adapt through changes in energy expenditure and appetite that make continued weight loss more difficult (PMID: 29156185).

For example, someone may be eating better than before, but sleeping poorly. Or exercising consistently, but losing muscle during weight loss. Or eating whole foods, but still consuming more energy than their body needs. Or dealing with insulin resistance, cravings, medications, chronic stress, digestive symptoms, or inflammation.

That is why a functional medicine approach can be helpful. Instead of assuming the same plan should work for every person, we ask better questions:

  • What is your weight history?
  • What has worked before?
  • What has failed before?
  • Are you losing fat, muscle, or both?
  • What do your labs show?
  • How is your blood sugar?
  • How is your sleep?
  • How is your digestion?
  • Are you hungry all the time?
  • Are cravings driving the day?
  • Are medications affecting weight?
  • Are you under-recovering?
  • Do you have a plan you can actually maintain?


This matters because the best weight loss plan is not necessarily the most extreme one.

It is the one your body can respond to and your life can support.

Why You May Not Be Losing Weight

There are many possible reasons weight loss can stall. Here are some of the most common.

1. You May Be Losing Muscle Along the Way

Weight loss is not just about losing pounds. It is about improving body composition.

When body weight decreases, we want to know what you are actually losing: fat, muscle, water—or some combination of all three.

This matters because caloric restriction alone can result in loss of both fat and lean tissue. Resistance training during weight loss can help preserve lean mass, improve strength, and promote more favorable changes in body composition (PMID: 35191588; PMID: 40909191).

Adequate protein intake is important here as well. Higher protein intake during weight loss may help preserve muscle mass, particularly when combined with resistance training (PMID: 39002131).

2. Your Blood Sugar May Be Working Against You

Blood sugar is not only a diabetes issue. It is a metabolic health issue.

When your body has trouble using insulin effectively, blood sugar regulation can become more difficult. Insulin resistance can contribute to prediabetes and type 2 diabetes and commonly exists alongside other cardiometabolic risk factors.

Some people struggling with their weight may also experience:

  • Hunger soon after eating
  • Belly fat accumulation
  • Elevated fasting glucose
  • Elevated A1C
  • High triglycerides
  • Difficulty maintaining weight loss


This does not mean insulin resistance somehow makes the laws of energy balance disappear. But blood sugar regulation can provide important information about your overall metabolic health.

Physical activity, resistance training, appropriate weight loss, and balanced nutrition can all improve insulin sensitivity and metabolic health.

For people with prediabetes, even relatively modest weight loss can matter. Losing approximately 5% to 7% of body weight alongside healthy eating and regular physical activity has been associated with a substantially lower risk of progressing to type 2 diabetes.

That is encouraging.

Small changes can matter. But the plan still needs to be personalized enough to work in real life.

3. You May Be Relying Too Much on the Scale

The scale is a tool.

It is not the whole truth.

It does not tell you:

  • How much muscle you have
  • How much fat you have lost
  • Whether your waist measurement is changing
  • Whether your blood sugar is improving
  • Whether your strength is increasing
  • Whether your energy is better
  • Whether your plan is sustainable

5. Sleep and Stress May Be Affecting Your Progress

If you are sleeping five hours a night, living in high stress, skipping meals, relying on caffeine, and trying to force intense workouts, weight management may become more difficult.

Sleep affects far more than whether you feel tired the next morning. Insufficient sleep can influence appetite, food intake, glucose regulation, recovery, and decision-making.

Stress can also influence eating behavior, sleep, recovery, and the habits required to consistently follow a plan.

This does not mean cortisol or stress is “the reason” everyone struggles to lose weight.

It means stress and sleep deserve to be part of the conversation.

Sometimes the most important next step is not a harder workout or stricter diet. Sometimes it is improving sleep, building a more realistic schedule, reducing chaos around meals, and making the plan easier to repeat.

6. Medications or Medical Conditions May Be Playing a Role

Some medications and health conditions can affect appetite, energy, fluid retention, body weight, or weight management.

That is one reason it can be helpful to work with a clinician instead of trying to troubleshoot everything alone.

If you feel like your body changed suddenly, your weight gain does not match your habits, or you are experiencing symptoms like fatigue, digestive issues, abnormal hunger, poor sleep, mood changes, or menstrual changes, it may be time to look deeper.
